Working from home maybe? I guess it may depend on the individual in regards to your symptoms as well as your skills. Maybe even a career that forces you out every once in a while to help you overcome this monster. Just do not confront your fears or push your self out there too far too fast or it will push you back in deeper. Speaking from experience...
Personally, I am daytrading. Sounds like a panic attack waiting to happen right? It's better than having to be forced out when I am not sure how bad I will be feeling and having to deal with people and not being able to function properly in the work place. I would not recommending that by any means as it is very tough "not to lose money" lol. I am just personally speaking-this is very very risky and takes a lot of courage. Daytrading is more or less 70% psychological than actual skill if that makes sense.
Use your creativity:
-I also sell my drawings and paintings on line and freelance every now and then for graphic design firms and marketing companies. Check out some courses and certificates available on-line for schooling.-Writing (freelance)? Maybe for birthday cards and such or marketing companies? You could do some research on what companies would purchase???
-Also, some Telamarketing and Customer Service positions are out there as some companies will now hire you and set you up to work from home if you do not mind chatting all day.
-Maybe Data Entry from home if you can type?
-For extra cash you can get your car wrapped with a companies logo if you do not mind being a driving billboard. Have not looked into it much but I would think location and type of car you drive would come into play. I read that it can pay pretty decent.
Please, always, always research the company you plan on dealing with over the Internet! I cannot express that enough. You do not want to get caught up in a scam but there are legitimate companies out there.

I feer just an interview at the moment but maybe confronting that and getting past it would be a good start. Maybe just interview for anything to get practice and comfortable? Just thinking out loud....
Hope this helps joggle your mind a little and maybe think of a path that suits you. Do some research and maybe you will stumble upon something that meets your criteria.
